Services Support - Support Teacher Transition
|

|
The
Support Teacher Transition assists schools in the Bankstown District
to support the transition needs of secondary aged students with identified
disabilities in both special and regular schools by: |
|
- Supporting schools in developing and implementing transition
planning processes.
- Working collaboratively with school and district staff, families
community personnel to facilitate the transition of students from
school to post school.
- Attending Individual Transition Planning (ITP) meetings where
appropriate.
- Supporting schools, through training and development to establish
effective programs, which will assist students in their transition
from school to adult life (credential and pathway options in Stage
5 & 6, Life Skills, School & TAFE VET).
- Supporting schools in the development, monitoring and evaluation
of school based funding submissions (Link Support Funding, School
& TAFE Delivered VET).
- Assisting district committees with applications from schools
for transition and VET support in accordance with State guidelines.
- Working collaboratively with district officers, schools and
government and non government providers to assist students in
the transition from school to post school services (EXPO's, Transition
Directory, community networks, information sessions).
- Lliaising with schools, parents, government departments and
community service providers and other agencies on local issues
regarding transition.
